ABSTRACT

Objective: Orbitoplastic with microvascular free flap at complicated craniofacial injury, caused by mine explosion was presented as definite solution.
Clinical presentation: Two schoolgirls were seriously injured - bad craniofacial damages with loss of both eyes. Orbital - etmoidal part of one of the skulls was damaged and it caused orbital cerebrorrhoea.
Intervention: Urgent craniotomy with epidural hematoma evacuation, bone reconstruction and dural replacement, the reconstruction orbital roof and anterior skull basis of were performed in the first part. The second part consisted of orbital plastic with ulnar microvascular free flap.
Conclusion: This serious case of air explosive craniofacial injury was sucessfully completed by orbital plastic with ulnar vascular free flap, suggesting itself as a good choice in similar situations.

Key words:: craniofacial injury, orbital cerebrorrhoea, neurosurgical treatment, microvascular flap.
